Toddler House No 6

It was determined that the operation failed to maintain supervision.

Based on information gathered during the investigation there is sufficient evidence that a child in care was hit with a ruler by the caregiver.

During the walkthrough I observed a waterhose on the ground. This was not corrected due to the operation not being able to remove the hose from the water main line in the wall.

The operation has failed to maintain 9 inches of loose filled material for equipment greater than 5 ft.

During review of personnel 2 out of 7 records evaluated did not have a notarized affidavit.

During review of Personnel 4 out of 7 records evaluated did not have current first aid and CPR with rescue breathing. 3 caregivers were found to have online training. 1 caregiver did not have Infant and Child CPR as required.

During review of records 6 out of 10 records evaluated did not have all required information for the emergency contact.

During review of records 1 out of 10 records did not have a phone number for the release information.

During the walkthrough the caregiver was not able to give me the ages of 4 chldren combined in 2's classroom and did not have information showing their ages.

During review of record keeping 2 out of 10 records evaluated did not have a health statemtent.

It was determined that the caregiver was using prohibited punishment when she made children do wall sits as a form of discipline.

It was found that an infant was left in the car seat for an hour resulting in the child getting anxiety and scratching his face.

It was found that infant/toddlers bottles are not labeled and children are being fed from the wrong bottle.

It was found that diapers are not promptly being changed

It was found that children?s belonging are lost and improperly stored/placed in the wrong backpacks. This include diapers, bottles and clothing.

One out of seven personnel files reviewed is missing the transportation training. Note: The caregiver regularly does transportation.

One out of seven personnel files reviewed is missing the health and safety training. The caregivers training year is 06/2020 to 06/2021.

During the inspection it was found that one infant (4 month) did not have the infant feeding instructions signed by the parent for the month of October.

During the inspection it was found that the outside area did not have proper surfacing under the play structure and swing. Note: Pictures were taken.

One infant did not have a signed and completed feeding intruction.

There was only one crib labeled in the infant room.

There was a twenty month old in the same infant room as a 7 week old infant. This was corrected when the director moved the older infants to another class.

The posted CPSC form is not current, it is date for 7.23.2020.

There are several ceiling tiles in all classrooms that appear to have water spots on them and in the hallway there is a ceiling tile that has a hole in it.

Ants were found in the infant room on the shelf that the formula is kept. The windowsill in the infant room was not clean, it had dust, bugs and other debris.

No daily information was being shared with the parents.

The insurance information on file expired on 6.3.2021.

There are several areas of exposed sheetrock in the Green classroom that are accessible to children.

The director has not been at the opeartion since 7.12.2021. No other director qualified person to be left in charge.

The outlets in the two year old classroom did not have outlet covers and were accessible to the children.
