During the walkthrough I observed a waterhose on the ground. This was not corrected due to the operation not being able to remove the hose from the water main line in the wall.
The operation has failed to maintain 9 inches of loose filled material for equipment greater than 5 ft.

During the walkthrough the caregiver was not able to give me the ages of 4 chldren combined in 2's classroom and did not have information showing their ages.

It was determined that the caregiver was using prohibited punishment when she made children do wall sits as a form of discipline.
It was found that an infant was left in the car seat for an hour resulting in the child getting anxiety and scratching his face.

During the inspection it was found that one infant (4 month) did not have the infant feeding instructions signed by the parent for the month of October.
