Based on the interview with children there is enough evidence that three children were hit by a caregiver. Caregivers involved are no longer with the operation.
Caregiver did not demonstrate good judgement when she decided to hit a child with a belt even though child said he asked for it.
Three caregivers in each of the 3 classrooms did not know how many children they had in care. The caregiver in charge of the younger group 5-7 stated there were 20 children, but he was not sure how many children he had because some had left. The caregiver then counted the children while I was present. The caregiver in charge of the group ages 8-10 counted the children while I was present and the caregiver in charge of the group ages 5-11 had to look at the sign in log to count the children.
Two caregivers in charge of a group of 17 children ages 8-9 years were not aware of the number of children in their care. One caregiver stated there were 14 children present and the other caregiver stated there was either 15 or 16 present.
During the walkthrough, a caregiver was observed to be in care of children with notification of eligibility from the background check unit. NOTE: Background check was inactivated 1/8/2021
The operation failed to use good judgement by not following all conditions of a safety plan that was implemented during an investigation. The Director of the operation also failed to use good judgement by not notifying staff that there was no one available at the operation to care for children when they were dropped off after school.
There is sufficient information to determine the operation did not share an incident report with parents of children involved in an investigation.
Based upon the information obtained from the investigation, there is sufficient evidence to support a child(ren) were neglected.
