The director and two caregivers did not have CPR training that was conducted in person.

One caregiver had 15 hours for training year 4/5/2022 to 4/5/2023. Corrected: When 9 hours were taken from training year 4/5/2023 to 4/5/2024.

Caregiver was changing children at the changing station and three infants were in the classroom. Another caregiver was in the restroom assist a child when another child step away to another classroom.

On the playground parts of the fence is missing causing a hazard to children to stick their head between the boards. They were broken boards on the fence and the lock on the gate is broken.

One child is missing immunization records.

The operation was missing how they would evacuated or relocated children 24 months and younger.

The caregiver stated she had 10 and I counted 9 children.

A child with a food allergy didn't have a food allergy plan on file.

Three caregivers are missing their 24 hours of annual training.

One caregiver in a class did not know the correct ages of all children nor could reference the ages from documentation. Note: this was addressed at inspection when the director corrected the caregiver with correct ages.

A caregiver is missing the employee affidavit form in their personnel file.

A caregiver is missing a photocopy of their driver license and social security card.
