During the review of children's records 2 out of the 4 records evaluated was found to be missing the emergency contact address. Corrected at Inspection: A caregiver reached out to the parents and obtained the addresses for the two emergency contacts listed.

During the review of children records 2 out of the 4 records that were evaluated were found to be missing the food allergy emergency plan.
The enrollment records for four children were evaluated during the inspection. I observed the following information missing from the files: 1. One child did not have documented information indicatiing whether or not he has any special needs. 2. One child did not have the following: A. An address documented for the emergency contact person. B. The name and contact information for their elementary school.** **NOTE: This information was corrected during the inspection.
I observed one caregiver working with 5 infants. The specified age of the group was 10 months and the required child/caregiver ratio for 0-11 months is one caregiver with 4 infants. NOTE: This was corrected during the inspection when one of the older infants was moved to the toddler classroom.
I observed that the background check status was still pending for a caregiver who was observed working with children during the inspection. NOTE: This caregiver was sent home during the inspection.
I observed that the SIDS, Shaken Baby Syndrome and Abusive Head Trauma training had expired for a caregiver who was observed working with infants during the inspection.
I observed that 3 of the caregivers working with children do not have current certification in first aid.
In two different classrooms, the caregivers did not know the ages of the children and there was not a list of children in the room with documentation of their ages.

During review of record keeping 6 out of 10 evaluated was found to not have all required Emergency contact information.

During the inspection 1 caregiver was found to have been caring for 24 children between 23 months and 11 years old. The SAG was 5 years. Ratio is 1:22 the caregiver was over ratio by 2. This was corrected when a caregiver took the younger children to their classroom.
A caregiver was not able to provide the age of a child in class and did not have information showing age.
During the follow up a caregiver stated she was caring for 16 children when she was found to have 24 children in classroom.
During the walkthrough a caregiver stated she was caring for 14 children between 3-6 years it was later determined that she was caring for 15 children between 2-6 years.
During the walkthrough I observed a 14 month old asleep in a restictive device.
During the walkthroughI observed a 6 year old in the front office area unsupervised. I observed 15 children between the ages 2-6 years old unsupervised during nap time and 8 schoolers in a classroom without a caregiver.
During the walkthrough I observed a caregiver place toys that were in a childs mouth back into a bucket without setting aside.
During the walkthrough a caregiver was not able to provide the ages of children they were supervising and did not have information to refer.
