Based on the information obtained from the investigation, there is sufficient evidence to confirm that the caregiver in Schoolers Classroom was yelling, cursing, and calling children names while supervising them at the operation.

Based on the information obtained from the DFPS investigation, good judgement was not implemented when a school age child was placed in the school age classroom for redirection purposes although there was a adult not responsible for a group of children they sent them with the assistant director whom was in the infant classroom. It is also confirmed that a caregiver was using their personal cellphone to send text messages to another employee informing them of an altercation at the operation instead of possibly intervening it.
Based on the information obtained from the DFPS investigation, the incident report form 7239 was not completed properly for the incidents occurred on 10/24/2021 when a child sustained an injury on the playground and a child was throwing a baby bouncer toward a caregiver and the infants in care.
