Based on the information obtained from the investigation, there is sufficient evidence to confirm that the caregiver in Schoolers Classroom was yelling, cursing, and calling children names while supervising them at the operation.

Based on the information obtained during this investigation, child was sent home with soiled shorts.

The personnel and training records for the employees was observed not being maintained.

The infant feeding intructions for one infants was observed not current, last certification date was noted 1/3/22.

The annual training for the director of the operation was observed missing 11 hours of training under the following topics as specified in subchapter 746.1311 of Minimum Standards.

The First Aid kit was observed not available in one transportation van at the operation. This was corrected, a first aid kit was added to the van during inspection.

An 8 month old Infant was observed sleeping on the electrical bouncer during the walkthrough of the inspection.

Based on the information obtained from the investigation, Child Care Regulatory office was not notified regarding the positive cases for COVID for the employees at this operation.

Based on the information obtained from the DFPS investigation, supervision for children in care was not maintained when a caregiver left the children unattended.

Based on the information obtained from the DFPS investigation, a school age caregiver did not interacting with children in a positive manner when the caregiver was cursing at the children and threated to whip them. It is also confirmed that a caregiver was humiliating a child by calling them names.

Based on the information obtained from the DFPS investigation, a caregiver in charge when the director not present did not meet their task required when additional information requested for the investigation was not provided timely. It is also confirmed that parents were having difficulties reaching out to the operation when the director is out of office.

Menu was observed not posted in the front lobby of the operation during the inspection. This was corrected, the menu was posted during the inspection.

Based on the information obtained from the DFPS investigation, good judgement was not implemented when a school age child was placed in the school age classroom for redirection purposes although there was a adult not responsible for a group of children they sent them with the assistant director whom was in the infant classroom. It is also confirmed that a caregiver was using their personal cellphone to send text messages to another employee informing them of an altercation at the operation instead of possibly intervening it.

Based on the information obtained from the DFPS investigation, the incident report form 7239 was not completed properly for the incidents occurred on 10/24/2021 when a child sustained an injury on the playground and a child was throwing a baby bouncer toward a caregiver and the infants in care.

Based on the information obtained from the DFPS investigation, the operation violating this standard by placing a school age child 10 years old in the infant classroom on 10/24/2021.

Based on the information obtained from the investigation, supervision was not maintained when a child was confirmed occasionally run out of the classroom without the primary caregiver's permission. It was confirmed that there were several times the child ran out of the classroom, and the primary caregiver was not aware.

Based on the information obtained from the investigation, parents were not informed nor allowed to come inside the daycare center after the Emergency Rules were lifted.

4 out of 6 classrooms failed to have a cover sheet over the Food Allergy List to protect confidentiality. Note: this was corrected at time of inspection when the lists were covered.

1 infant under the age of 12 months old, was sleeping in the crib with a bib around the neck. Note: this was discussed at time of inspection with the director

3 classrooms with children under age of 2 years old had a wall mounted tv playing children's programming. Note: this was corrected at time of inspection when the caregivers turned the tv off.

2 children were sitting in a high chair without safety straps fastened. Note: this was corrected at inspection when the caregiver fastened the safety straps.

3 caregivers did not know the ages of the children they are in charge of and could not reference the ages in the class. Note- this was discussed with the director.

There is sufficient evidence to support the fact that a chair was thrown by one student towards another student.

Some of the fasterns for the playground's border are starting to rise out of their socket.

Two of the student files did not have an address for their emergency contact. This was corrected when the director called the parents to obtain the address for the emergency contact.

One of the student files was found to be missing their physician information. This was corrected when the director obtained the physician information from the child's parent.
