Based on information obtained throughout the investigation inappropriate discipline was practiced when a caregiver used physically discipline.

I observed a 2 month old in a crib with a bib that was not on the child's neck and a pacifier clip on a string with beads. This was corrected when the caregiver removed the items from the crib.

A caregiver provided the incorrect age of a child. This was corrected when the director informed the caregiver the correct age of the child.

Caregivers did not provide the correct ages of the children they were caring for when asked.

There were three people in the building without active background checks. One of the people without an active background check left the operation.

There was no documentation of the smoke detectors being tested.

There were no fire drills, lock down drills, or severe weather drills documented at the operation.

There were two children under two watching TV.

No one knew who was in charge when the director stepped out.

During the first initial phase and during the first initial inspection for the second initial phase the director has not been present for the visits.

The operational policies is missing the section on promotion of physical activity and procedures for supporting inclusive service.
