Based on information obtained throughout the investigation inappropriate discipline was practiced when a caregiver pulled a child across the floor causing redness to appear on the child's rib cage.

During the walk through in one classroom the caregiver did not know the number of children they were responsible for. Caregiver stated 11 and there was 12 children.

Based on the DFPS investigation and after reviewing the camera footage, it was found that the caregiver was not supervising the children. Children were observed climbing on tables, a child pulled a chair that was holding the restroom door open, and the child inside was left in the restroom and the caregiver was not interacting with the children.
