The provider failed to adhere to two (2) conditions placed on their operation during the corrective action period: (1) The probation notice was not posted in a prominent place near all public entrances of the operation as required; and (2) Documentation was not available for review during the inspection to support a meeting was held with staff informing them the operation was on probation and if they had suggestions on how to prevent the deficiencies.

On the day of the inspection, the home was over ratio and capacity. The home has three bedrooms, one of the doors was locked and licensing was not able to inspect the room to ensure additional children were not in care.
It was determined the caregiver in charge of a group of children did not demonstrate Competency by not being aware a two year old child was in care and inside of the closet in the back bedroom. The caregiver stated the child the primary caregiver's relative and she didn't realize the child was present.
Based upon the deficiencies cited during the follow-up inspection along with the weights of each standard cited, it was determined the permit holder does not operate in compliance with the minimum standards. On the day of the inspection, the home was over capacity, over ratio, a child was hiding in the back closet, and a bedroom door was locked inaccessible for licensing to inspect.
A 14 year old was identified as someone who was helping out.
