The operation did not have an up to date fire inspection. The most recent fire inspection was completed on 9/13/2023.

I evaluated 10 childrens records and the following was observed: - One child did not have an emergency care authorization - One child did not have a phone number or address for the emergency contact. - One child did not have record of immunizations - One child did not have an up to date record of immunizations

During the inspection I observed 12 children to be present in the infant room. Based on the specified age group, the room exceeded the maximum group size of 10. NOTE: During the insepction, two children were moved to another classroom to maintain ratio.

One caregiver with a pending background check was observed to be present in the classroom and caring for children. NOTE: This was corrected at inspection when the caregiver left the operation.

In the toddler class, cough medicine was found in an accessible small storage bin drawer. The cough medicine was in the 3rd drawer from the bottom which is reachable for the toddler children. This was corrected when the director removed the medicine from the drawer and placing it in the office.

During the walkthrough one toddler room did not have the fire exit diagram posted. Note: This was corrected at iinspection when it was posted.

Three infants were observed sleeping in the crib with blankets, bottles and bibs. 5 month old had a bib on and bottle inside the crib. 10 month old had a blanket inside the crib 10 month old had a bottle inside the crib.

One infant was observed sleeping in the crib with pacifier cord attached.

One toddler classroom did not have the nap chart showing which cot belonged which child. Note: This was corrected when the nap chart was made.

One caregiver present in the infant room did not have valid background checks. The background check is inactive. Note: Caregvier was asked to leave.

One out of ten child records reviewed is missing the immunization record on file.

It was observed that in the toddler room a cot being used by a toddler was blocking the exit.

Eight out of ten caregivers files reviewed is missing the educations qualifications on file.

The fire drills for september was not documented.

One infant was observed asleep in a restrictive device (Bouncer).

One out of ten child records reviewed is missing the health statement on file.

A classroom with 19 children in care ages 3-4yrs old was observed napping without an adult supervision.

5 month Infant was observed sleeping in crib with a pacifier attachment and bib around her neck.

During investigation food allergy plan for child enrolled with a food allergy could not be observed.

During investigation Director was not aware of child with a medical statement declaring a child food allergy.

During investigation a list of children in care with food allergies was not observed.

3 caregivers were observed caring for children with an active background check.

2 infants 12months and younger were observed sleeping with blankets, wipes and pampers in crib.

During follow up 3 of the 6 caregivers folder did not have clock hour trainings.

2 infants were observed in high chairs with safety straps unstrapped.

A caregiver was observed in the Infant classroom without an active background. Corrected at inspection: caregiver was asked to leave.

There were 2 caregivers observed with 9 infants in care age ranges from 4-12months.

Mulch was not observed on the outside play area.

Current annual fire inspection could not be observed. Fire inspection observed dated 7/25/2019.

3 of the 6 caregivers folder observed did not have clock hour trainings.
