Based upon the information gathered during the investigation, one child was left unsupervised in a classroom for approximately 6mins on 4/3/2025.

During the inspection I observed a caregiver not washing their hands after wiping a child's nose. Caregiver washed their hands after it was brought to their attention by the inspector.

During the inspection of the outdoor play area I observed a torn net on a basketball goal, I observed a hole approximately 2 feet deep as well as broken teeter totter leaning against the gate. During the inspection the net was removed from the goal and cones were placed around the hole until it could be repaired.

During review of naptime I observed 2 children's cots blocking the backdoor exit. This was corrected when the caregiver rearranged the cots.

One child, two years of age was left in the classroom unattended for approximately 5 minutes. Three caregivers were outdoors while the child was left unsupervised in the classroom. The caregivers were not in physical proximity and lacked visual awareness of the child during the time of the incident. The child was observed/obtained by another staff at the childcare facility that was not considered the designated caregiver.

It was not good judgment of either caregiver when they did not supervisor children because one caregiver was doing children's hair and the other caregiver was talking to other staff on another playground.

The two children's incident report was missing directors name, operation number, it did not have documentation if parents or licensing was notified, details of the incident, the child?s date of birth, the caregiver that was in charge or the additional staff that was present.

According to caregivers they were not adequately supervising children when they were able to walk back into the building without either caregiver noticing the children were gone for approximately ten minutes.
Based upon the information obtained during the investigation, two caregivers responsible for a group of children, left a child outside on the playground alone. The child was outside alone for less than 5 minutes before a caregiver from a different classroom noticed the child was outside alone.
