Toilets, sinks, doors and floors are not being kept clean. The toilets have rings, a toilet brush was observed on the floor in the restroom during the walkthrough. Doors and floors have stains and dirt throughout the daycare.

During the inspection it was observed that the diaper changing mat had tears, exposing the foam. Note: This was corrected when a new one was ordered.

During the inspection it was observed that a new hire was in the classroom observing with a pending background check.

The first aid kit contained expired alcohol wipes; which expired in 2017. Note when brought to the director's attention expired wipes were removed and replaced with wipes that were not expired.

During the follow up it was observed that the maintenance items (Such as the toilet and floor boards and outside entry way) were pending repairs. Note: A work order has been made to get everything repaired this week.

A child in the infant did not get their hands washed after having the diaper changed. This was corrected at inspection.

In one room the nap cot chart was missing. This was corrected at inspection.

During the walk through it was observed that one restroom had toilet paper clogged and toilet paper everywhere on the floor. The floors boards in the restroom and toddler room are broken and open. It was also observed that the outside entry way to the playground has dirt and water making hard for children to cross inside.

One employee in the infant room did not wash hands after changing diaper. This was corrected at inspection.

During the follow up inspection it was found that one caregiver was counted in ratio with a pending background check.

One classroom did not have the roaster showing the name and ages of children.

During the inspection it was observed that the mulch (surfacing) is not to proper depth. It was observed that there is more dirt than mulch. Pictures were taken.

One child was observed sleeping in a restrictive device. Child was had been sleeping for 10 minutes. Note: This was corrected when the child was moved to the crib. Pictures were taken.

It was found that caregiver records are not available for review at the operation.

An infant was observed sleeping in the crib with the bib on. Note: This was corrected at inspection when the bib was removed.

One infant observed sleeping in the crib with pacifier cord attached. Note: This was corrected at inspection when it was removed.

A video was observed provided by Director, 2 children were seen unsupervised. There were a total of 3 caregivers in the building at the time of the incident. All 3 caregivers were observed doing other duties.

During the inspection it was observed that there was an rubbing alcohol located at the bottom of the cabinet and the cabinet was not locked. Note: this was corrected when the cabinet was locked.

The food allergy plan is missing for one child with diagnosed food allergy to eggs.

The operational policies is missing information on gang free zone.

The emergency preparedness is missing information on how to contact the local authorities.
