There were three outlets that did not have outlet covers in the 22-36mo old room. The Director corrected this at the time of the inspection by covering the outlets.

There were two classrooms that there was not enough adequate lighting for all the children to be seen in the event of an emergency. The Director corrected this at the time of the inspection by opening covers over windows so the rooms had enough lighting.

DFPS investigator received information to support a child in the care of the operation was neglected.

A caregiver in charge of the supervision of a 3 year old group was not properly supervising the group of children on the operation's playground. This resulted in a child leaving the playground area and crossing a highway. A passerby intervened and brought the child to a nearby store.

There was a trash can in a restroom that did not have a lid, which made it to where garbage was accessible to children. The Director corrected this at the time of the inspection by replacing the trash can with one with a lid to prevent the spread of germs.

Based on the information gathered and seen during the inspection, there was one caregiver in the building with six children 2-3 years old in the classroom who had to leave the children unsepervised several times to screen children and take them to the room. This was also seen on the sign in and sign out logs for the week of 4/5-4/9. The Director is going to correct this by having a supervision training and having another staff member come in at opering so no children are ever left unsupervised.
