Violation Details
Based on the findings of a DFPS investigation a caregiver did not intervene to ensure safety by picking up a barefoot child or by stopping to put the child's shoes on instead of continuing to walk across hot pavement when they noticed the child's shoes were not on. The child's feet sustained burns. Prior to transitioning from the playground back to class supervision was not maintained on the playground as caregivers sat down on the playground and were not walking around and intervening where appropriate. They were not ensuring this child's shoes (and others stayed on) or seek assistance. More than one child walked back to class without shoes on when transitioning from the playground back to class.

I observed an infant sleeping in a crib with a bottle in it. Note: This was corrected during the inspection when the bottle was removed.
During the time of the incident where a child was left alone unsupervised, the caregiver did not have access to attendance tracking information electronically or on paper. During the first hour of operating hours at the center, caregivers do not have tablets to access the sign in information electronically or have paper sign in sheets as children arrive to the classrooms. While the caregivers track the number of children the first hour of the day, there is no tablet or paper roster with the children's names or time of arrival. Management delivers rosters to the classroom every day after the first hour of the day when the drop off cut off time is.
A child was left alone unsupervised in a hallway outside their classroom for approximately 20 minutes.
A new caregiver did not have current first aid at the time of an injury of a child in her class. She was the sole caregiver of the class.
