During the monitoring visit, there were four infants that were not buckled in their chairs that were connected to the table. The ages of the children were 16 and 17 months. The caregiver buckled the four infants chairs upon request during the monitoring visit.

During an investigation inspection, three caregivers were observed in the operation and caring for children without an eligible background check.
During the inspections, pack n plays were observed touching eacher other with no walking room between and allowing infants to have contact with other cribs. The beds were moved at inspection coming into compliance.
During inspection, children's mats were next too each other in rows of four or five with no walking room between them.
During inspection i observed one caregiver with 11 infants and one caregiver with 6 infants.

Throughout the inspection I observed 3 different infants asleep in a bouncer for more than 10 minutes each and the caregivers had opportunity to move them.
A caregiver is present and caring for children with a pending background check. Staff have stated that the caregiver started working at the center on 10/18. The caregiver's background check was submitted on 10/11. This was corrected at the inspection when the caregiver left the operation.
The three and four year old classroom has an activity plan posted showing their daily schedule. Upon entry of the classroom, this hadnt been completed. This was corrected during the inspection.
The playground structure has four nails sticking out of the structure where children climb up to. Also the slide has torn cuts that could cause a tear in the skin while sliding down.
The most recent Gas leak test was done in January 2019. This is required to be conducted every 2 years
The caregiver was unable to provide documentation of CPR first aid certification completion. The certifications provided expired in september 2021.
The three caregivers who transport children do not have current certification to transport children. The caregivers are required to have 2 hours annually.
The three and four year old classroom does not have an activity plan posted showing their daily schedule. The caregiver stated verbally what the children do hourly.
