The caregiver CPR training was completed on line.

During the monitoring visit, there were four infants that were not buckled in their chairs that were connected to the table. The ages of the children were 16 and 17 months. The caregiver buckled the four infants chairs upon request during the monitoring visit.

2 out of 8 children were missing the name, address, and phone number of the children's physican.

6 out of 7 staff records reviewed did not have 24 hours of annual training.

The playground structure needs to be repaired. It has loose nails accessible to children. Wooden pieces are broken and can cause splinters to children.

All play yards had extra mattress space.

All infants in the infant room were missing infant feeding instructions.

The director did not have 30 clock hours of annual training.

Infant caregiver was seen talking on personal device while infants were in care.

All of the play yards were missing the child's name or number for labeling.

On the playground structure, it was missing a wooden piece that creates a opening for entrapment.

During the investigation, it was determined that the caregiver did not wash hands after completing a diaper change.

During the investigation it was determined that the operation failed to maintain ratio.

During the investigation it was determined that a 7-month-old child was asleep in the play yard with a toy in the play yard.

The director transports four children and does not have a list of children she is transporting.

The director does not have emergency information on the children she is transporting.

The director does not have current transportation training.

The infant classroom was observed watching television.

A child was observed using screen time and is under two years old. This was corrected when the television was turned off.

I observed 18 children present with one caregiver. The maximum amount of children with this specified age group may supervise is 15.

An employee was observed changing multiple children's diaper without washing hands.

The operation does not have a list of children that are transported.

Four employee records were reviewed and are missing the following information: #1 and #2 are missing affidavit of employment #2 statement about parent handbook and employee handbook as well as verified that she was provided orientation.

Four personnel records were reviewed and the following training is missing for three of the staff: MK needs 14 more hours of training for the last full year DB needs 24 hour for last full year LB Nees 24 hours for last full year

Four staff training records were reviewed and the following training is needed for MK, DB, LB: Abuse/Neglect Training SIDS/Abusive Head Trauma/Brain Dev Emergency Preparedness Medication Training Building and Premises Safety Training Communicable Diseases Training Food Allergy Training Hazardous Materials Training

Four children's records were reviewed and the following information is missing: Child # 4 needs child's address Child #1, #3 and #4 need address and phone number of emergency contact Child #1, #3, and #4 need phone number for their release persons information Child #1 needs transportation, field trip, and water permission boxes checked on front page

Four caregiver training records were reviewed and MK, DB, and LB need these trainings for their last full year. These trainings cannot count toward the current year.

Four children's records were reviewed and are missing the following: Child #3 is missing health care information on admission form and also the date the parent signed the emergency authorization part of the form.

At follow up torn mats were still being used for naptime. During inspection, i observed two children sharing one mat. This was observed with two mats. 20 mats were observed to be torn and can no longer be santizied.

During an investigation inspection, three caregivers were observed in the operation and caring for children without an eligible background check.

During the inspections, pack n plays were observed touching eacher other with no walking room between and allowing infants to have contact with other cribs. The beds were moved at inspection coming into compliance.

During inspection, children's mats were next too each other in rows of four or five with no walking room between them.

During inspection i observed one caregiver with 11 infants and one caregiver with 6 infants.

A caregiver was observed changing a child's diaper and putting the child in their crib without washing the infants hands or the caregivers hands and then picking up another infant.

During inspection, i observed two children sharing one mat. This was observed with two mats. 20 mats were observed to be torn and can no longer be santizied.

Throughout the inspection I observed 3 different infants asleep in a bouncer for more than 10 minutes each and the caregivers had opportunity to move them.

A caregiver is present and caring for children with a pending background check. Staff have stated that the caregiver started working at the center on 10/18. The caregiver's background check was submitted on 10/11. This was corrected at the inspection when the caregiver left the operation.

The three and four year old classroom has an activity plan posted showing their daily schedule. Upon entry of the classroom, this hadnt been completed. This was corrected during the inspection.

The playground structure has four nails sticking out of the structure where children climb up to. Also the slide has torn cuts that could cause a tear in the skin while sliding down.

The most recent Gas leak test was done in January 2019. This is required to be conducted every 2 years

The caregiver was unable to provide documentation of CPR first aid certification completion. The certifications provided expired in september 2021.

The three caregivers who transport children do not have current certification to transport children. The caregivers are required to have 2 hours annually.

The three and four year old classroom does not have an activity plan posted showing their daily schedule. The caregiver stated verbally what the children do hourly.
