The foster home had missing floorboards in the upstairs areas of the home and carpet on the top of the stairwell was not attached posing tripping hazards.

There was clutter throughout the home as well as items throughout the home on the floor. There were unclean dishes in the entire sink and on the counter. A bug was observed on the dirty dishes.

A child in care was observed to have the smell of urine during an inspection contact. The caregiver indicated the odor was due to a soiled diaper. The child was not changed over a two-hour period.
