On the playground I Observed 2 ride on cars that are missing their wheels. I spoke with the director about not using equipment if parts have fallen off as it is no longer able to be used according to manufacture instructions.

Observed 18 children in one classroom. The youngest infant was 4 months old and the oldest child in the room was 5 years old. The operation opens at 6:30 am and all rooms were still combined into one room at 9:00am. This was corrected when the infants were taken into the infant classroom and the other children were split into their classrooms.

Observed 2 slides on a mulched area that does not have a 6 foot use zone for either slide in front or behind the slide. The squared off mulch area, does not have enough square footage to have slides and meet the 6 foot use zone requirement.

A Caregiver left the toddler classroom with 5 children in it, to go to the office to make a copy of the Licensing inspectors badge. There was no other caregiver in the classroom during this time. I spoke with the caregiver about supervision and not leaving the classroom

There were protruding nails, exposed wires, tall grass and broken plastic toys observed on the outdoor playground of the operation. The broken toys were removed during the inspection.
There were 2 infants sleeping in bouncy seats. This was corrected when the infants were removed from the bouncer seats.
Observed a infant sleeping with a bottle in the crib. This was corrected when the bottle was removed.
The manufactures instructions for a highchair were not followed which resulted in a child falling out of the highchair and receiving an injury.
A caregiver was using cell phone when in the role as caregiver for a group of children in care.
The operation reported a positive covid case by email on a weekend which when licensing recieved the report Tuesday morning, the 48 hour reporting time period had lapsed. This was corrected when the correct way to report was discussed with the operation staff.
It was found that an allegation was made on inapropriate discipline and it was not reported to licensing.
Observed a slide on the playground without loose fill. This was corrected when the slide was removed.
The operation was caring for children without a permit with the State.
