Observed a toilet in one room that was unable to be flushed which caused the bathroom to have a strong smell of urine. Observed the bus with trash all over the floor and seats. Observed multiple toys on the small playground that was broken, and had sharp pieces that could pinch children or cut them. Obsesrved the GaGa pit that had a metal guide post that was sticking out and can cause a child to fall and get injured on the guide post. There was a sheet on one of the cribs that had a rip in the sheet.

Observed one classroom have a shoe organizer attached to the back of a water heater room that did not have the ability to be locked and the chemicals were in the reach of the children. The door was not latching when shut and was able to be pulled open without twisting of the door handle. In the school age area, there were sunblock in a closet that did not have a lock on it and the sun block was in the reach of the children.

Observed a bathroom that had mats, toys and other equipment that stacked half way up the wall and on top of the sink and toilet and could not safely be used by any children. This bathroom is also a emergency evacuation room.

Observed one classroom have lysol and other cleaning supplies in an unlocked cabinet. Observed a second classroom with Rubbing alcohol and peroxide in an unlocked cabinet. Both were removed from the unlocked cabinets and put out of the reach of the children in care.

Observed 7 infants sitting in the table highchairs and no straps were being used on any of the children. This was corrected when the children were strapped in to the seats.
Observed a caregiver cooling off a warm bottle in the handwashing sink. I spoke with the caregiver and director regarding cross contamination.
During the monitoring inspection it was found that at least 10 of children's files did not have an updated immunization record on file.
During a DFPS investigation, it was found that a caregiver neglected a child in care when the child was left unsupervised and able to walk home alone.

These caregivers also did not know the age range of the children in care. In a second classroom another caregiver did not know the ages of the children in the classrroom.
During my walkthrough of the classrooms, a child was found alseep in a high chair. The child was not fastened in the highchair with the provided straps.
A caregiver at the operation does not have the qualifications to be a caregiver. The caregiver is not enrolled in a child care career program.
It was found that a caregiver did not promptly change a child's dirty diaper on multiple occasions.
A caregiver used unusual punishment when placing a child on a stack of 4-6 chairs for time out, knowing the child could not get off the chairs without assistance.
A caregiver placed a small child on a stack of 4-5 chairs for time out. The caregiver did not use good judgement when placing the child on the stack of chairs, which could have resulted in a injury to the child if the child had fallen.
While reviewing the operations BrightWheel applications for the infants daily reports, a child in care did not have a daily report completed on 8/17/2021. Also on 8/18 the infant report only showed a snack and nap time. On 8/19 the infant report showed 2 diaper changes and snack. On 9/3 the infant report showed only diaper changes.
During my interviews with staff it was found that children have been placed in a high chair for time out.
A 4 month old infant was observed asleep in a restrictive device (bouncer). The caregiver removed the infant from the bouncer.
There were 2 children (12-18 months) observed in a seat that required a safety strap and the children were observed to not be fastened with the safety strap. The caregiver fastened the straps of the 2 children during the inspection.
A caregiver in the 4 year old classroom did not know the ages of the children she was caring for. The caregiver stated the information was not passed down to her.
In the 2 year old classroom, the group was split during transition and each caregiver did not know how many children were in their group.
