Observed lysol and frebreeze in an cabinet with a broken lock that was in reach of the children in the classroom. This was corrected when the chemicals were removed from the unlocked cabinet and placed in a locked cabinet.

Observed a caregiver hold a bottle to an infants mouth while the infant was propped up on a boppy pillow on the floor next to the caregiver. The caregiver stated that the child could not hold their own bottle. I explained that an infant who cannot hold their own bottle and sit unnassisted should be held to be fed. The infant was picked up by the caregiver.

Observed a metal vent screen that is broken and falls frequently due to not being ablt to be secured with the latches. One latch is bent and the other latch is missing. There is also a wooden board broken in half from the fence on the playground.

During an investigation, there was enough evidence to support that caregivers are yelling at children as a form of discipline.
The operation did not have a current sanitation inspection. The last sanitation inspection was completed on 1/23/23. The operation will be scheduling the inspection.
Observed pecans on the infant/toddler playground. I observed 2 infants with pecans in their mouths which can be a choking hazard. I spoke with the director regarding picking up the pecans prior to allowing the young infants/toddlers on the playground.
Observed a classroom, the caregivers stated that they had 13 children, but there was 14 children in the room.
During the inspection it was observed that an outdoor area was over space capacity by 6 children and an indoor classroom was over capacity by 2 children.
During the investigation it was found that ill children have been cared for around other children, increasing the chances of spreading illnesses around.
During the inspection it was found that children were allowed back into care before the were cleared from a physican to be present.
During the investigation it was found that there were multiple incidents where children were biting, hitting, kicking, choking, and pushing other children in care.
During a DFPS investigation, the walls in the classroom were observed to have peeling paint in multiple places.
During a DFPS investigation, a changing pad was observed to have multiple tears making it absorbent. This was corrected when the torn changing pad was replaced with a new intact changing pad.
The last fire inspection available for review was conducted in 2021.
There were 2 children records out of 10 with outdated immunizations. This was discused with the operations office staff.
Observed one caregiver on their phone while caring for children. Observed another caregiver with their phone on their lap. This was corrected when the caregivers quickly put the phones away when licensing entered the room.
Observed two classrooms that the caregiver did not know the ages of the children or how many children they had in their care. This was corrected when licensing spoke to both caregivers explaining the importance of knowing this information.
Out of 10 children files, 3 children did not have health statements.
It was found that a caregiver knew about an allegation but did not report it within the required 48 hour timeframe.
Observed 12 children sitting in high chairs and none of the children were strapped into the seats. This was corrected when the caregivers were asked to strap all the children in.
