The fire drills have not been documented. The last fire drill documented was May 2025. A fire drill was completed during the inspection today.

It was found that two caregivers at the operation have not been keeping a sign in/out log for the days/times that they are at the operation. I spoke with the operation staff about clocking in and out.

Observed two caregivers, both counted in ratio during nap time. There was not another person in the building that was not counted in ratio. I spoke to the person in charge and let them know that there must be one person not counted in the ratio at the operation during nap time. This was corrected when another person entered the building.

Observed a 12 month old infant sleeping in a basinet that states on the manufactures instructions that it is meant for infants 3 months old and younger and under 25 pounds. This was corrected when the infant was removed from the basinet.

Two classrooms did not have adequate lighting and licensing was not able to see any children when entering the rooms. Lighting was added when a curtain was opened a little.

Upon arrival at the operation, there were two pools outside on the playground. The operation failed to notify licening of the added services of adding a pool on their premises. I spoke with the operation about notifying licensing of any changes that are made for their services.

The operation added a swimming pool and a wading pool to their property and does not have the following safety devices that are required for an above ground pool: Life saving devices, inclosed fence around the pools, self closing latch or a lock out of the reach of the children on the door leading to the pools.
Observed Ibreprophen and a bottle of vitamins in the reach of the children, this was corrected when the medication was placed out of the reach of the children.
A classroom with 4 children ages 5 months to 2 years old was left unnatended when a caregier stepped into a different room to help a child. This was corrected when another caregiver came into the room.
Observed a wooden structure that had splintered wood which can cause children to get splinters.
Observed a 5 month old infant sleeping in a swing. This was corrected when the infant was taken out of the swing.
Observed chemicals in reach of the children in a classroom with children present. There was toilet cleaner on top of both toilets acceable to the children. In a second bathroom there was liquid fire, and hair spray under an unlocked sink. These items were removed and placed in a cleaning closet that children do not have access too.
During the monitoring inspection it was found that the last sanitation inspection was conducted on 03/23/2023. This inspection was overdue by 6 weeks.
During the monitoring inspection it was observed that there was some tools and working equipment, such as ladders and wheelbarrows, on the playground. Also on the playground was standing water in a child's wading pool, and a hose that was not rolled up.
During the inspection it was found that the operation's last fire inspection was on 04/21/2023. This inspection was overdue by 11 days.
During the inpection it was observed that the operation was caring for two infants and their permit did not allow them to care for infants.
During the monitoring inspection it was observed that an infant's diaper was changed and the caregiver did not assist the infant in washing their hands afterwards.
During the inspection it was observed that there were 13 children present, ranging in age from 13 months to five years old. The operation was over ratio by 1 child.
The operational policies were missing the following items; Hearing and vision screening requirements, Enrollment procedures, including how and when parents will be notifiedof policy changes, Promotion of indoor and outdoor physical activity, emergency preparedness plan, and How to contact licensing and the abuse and neglect hotline.
Observed the outside area with hazards. The operation had construction going on in the playground. There were large piles of dirt, drainage area was exposed which could cause a child to be injured.
