Observed a classroom that had 22 children in the room, the caregiver stated that they had 18 children in care.

Observed a slide and swings that did not have 6 inches of loose fill under or around in the fall zones. Some area's did not have any loose fill.
One caregiver had 7 children in care, There were 4 infants ages 0-11 months and two children who were 2 years old. This was corrected when the classroom was seperated.
Observed swings and slides that did not have the use zone to protect children from hitting other items. The Slide had about 3 feet from the slide to the boarder of the playground area and the stairs to the slide had about 2 feet use zone. For slides six feet high or less, the use zone in front of the exit of a slidemust extend at least six feet. The swings did not have the correct use zone in front or behind the swings. The use zone should be at least 12 feet. The use zone to the front and rear of to-fro swings (single-axis swings)must extend twice the height of the vertical distance from the swing beam to the protective surfacing below.
The operations activity plan states that breakfast is served at 8:00-8:30, This inspections started at 7:47am and the children had already eaten and the plates had already been returned to the kitchen area. I spoke with the director regarding following the schedule or making any necessary changes if needed.
Observed broken toys and bucket swing, trash and exposed screws from toys on the playground. The boarder to the loose fill was laying in the grass causing a tripping hazard.
Observed a 2 year old in the infant room with the youngest infant being 5 months old. There was at least a 19 month different between the oldest child and youngest infant in the room. This was corrected when the children were seperated into different rooms.
During an investigation it was found that the operation has been making breakfast at the operation without a food permit. It was also found that the operation has food made at an offsite location for lunch and this was not reported to the Health department.
The director was found to be lacking the required transportation training.
The last documented fire drill was conducted in July 2023.
One 2 month old infant was observed in a crib wearing a swaddling garment, restricting arm movement. The garment was removed during the inspection.
Observed a slide on the playground without loose fill. The slide was part of a playequipment that also had a swing. The swing part was detatched making the slide wobbly. This was corrected when the slide was removed.
Observed 5 children records, none of the children files had the Authorization to obtain emergency medical care and to transport the child for emergency medical treatment
Observed a caregiver caring for children without a cleared background check. This was corrected when the caregiver was asked to leave the operation. An administrative penalty in the amount of 100% of the daily maximum, per HRC 42.078(a-2) will be recommended as a result of this citation.?
Observed one caregiver's CPR card to be completed online. I spoke to the caregiver about having to use the hands on portion of the training for the training to be correct.
Observed one caregiver without pre service training.
Observed 3 staff files without proof of education or a notarized affidavit on file.
One caregiver stated that they had 6 children, but the caregiver had 8 children in care.
The operation had installed a swing without any loose fill under the swing. I spoke to the owner about the hazard it causes.
The operation had installed a swing to a tree which had a fence directly behind the swing which did not extend twice the height of the vertical distance from the swing beam to the protective surfacing below.
The infant classroom was left unsupervised when the caregiver came to answer the front door. The other classroom was left unsupervised when the caregiver brough a child inside and left 7 children outside without a caregiver. Both of these were corrected when the caregivers returned to their classrooms
The operational policies were missing the following information that is required like immunizations, parent notifications, outside play duration, ect.
The first aid kit was missing the thermometer.
