Ceiling tiles throughout the school are either missing, broken, or stained and need to be replaced. Walls need to be cleaned, especially door frames where handprints build up. Clean A/C filter metal cover to remove building up of dust and dirt.

During the investigation it was determined that a child received an injury while in care and did receive medical treatment. The daycare did not report the incident to licensing and complete the correct incident form.
