On the medication label the amount of Methyin is 10ml dated 4/2/2022, the medication log record is documented child received Methyin 5ml 1/26/2022 through 5/5/2022. The correct amount of medication 10ml was not administered until 5/6/2022 with the administrator's signature, date and time.
During the walk through of the infant room none of the infant bottles observed were labeled with the children's names.
During the walk through in the infant room a caregiver did not wash an infants hands after completing a diaper change.
