Operation failed to report a staff positive case to Health Department and parents within 48 hours of receiving the notification.

Operation failed to report a staff and child COVID cases within 48 hours after been notified. One case was reported 7 days later and the other one 3 days later.

Infant was observed sleeping in a bouncer. Between 3 to 4 minutes after approaching caregiver, infant was moved to a crib. form 2970 must be use to notify all parents within 5 days of receiving deficiency.

An ISD substitute teacher was caring for 3 infants at the daycare without a background check eligibility notification. The substitute teacher confirmed that she was caring for children when I arrived at the daycare and had to leave before I arrived at the classroom.

Director was aware that an ISD Substitute was caring for infants at the daycare without an initial background check. Daycare had 4 staff and was in compliance and did not need the ISD substitute.

An infant whose contract is as a drop off did not have a feeding instruction. Infant is using drop off services frequently.

A toddler and Infant caregiver were allowed to be present in the classroom without receiving background eligibility notification. The caregiver was asked to leave facility. Director took over the classroom.

Infant caregiver was feeding a 4 months baby while baby was sitting on a bouncy. Caregiver was approach while I was in the classroom. Caregiver was informed about minimum standards.

No documentation proving that smoke detectors were tested during the month of August and September was observed.
