A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

Three children's records did not have the updated immunization record on file.

A volunteer was seen to function in a caregiver role. The volunteer was unsupervised when she took children to the restroom and was in charge of the toddler classroom. The volunteer requires a background check to be submitted with FBI fingerprints.

Two caregivers did not have the pediatric first aid completed priorto direct care of the children.

There was no written documentation that the parents were notified that the operation did not have liability insurance.

The children's records reviewed did not have the date of admission on it.

There was no playground maintenance checklists available for review.

The personnel records were incomplete. The records lacked affidavit, educational requirements, training, employee statement re: personnel and center policies, and orientation statement re: abuse/neglect training.

The results for a volunteer, who was present in direct care of the children, had not been received.

The first aid kit was incomplete. The following items were missing: adhesive tape, cotton balls, scissors, thermometer, and tweezers.

The operational policies were incomplete and/or lacked all the required information, reference standard 746.501. The operational policies lacked 746.501(8, 9, 11, 12, 13, 19, 21, 22, 23, 24, 25, 26, 28, b 1-2).
