Based on information gathered during the investigation, a hazardous item, a broken leg chair, was placed at the playground and caused the incident to the child.

A group of children was left unsupervised when she stepped away from the classroom to assist inspestor at the front office.Children were left unsupervised in a classroom . Note: I discussed with the director about this situataion during inspection time.

The operation did not have Fire Drill Monthly practice to review during inspection time.The last one was on May 2022.

The operation did not have current sheltering drill for severe weather drill to review during inspection time.The last practice was on Feburary 2022.

There were 2 caregivers who were present and have been in direct care of the children prior to today. They did not have the background check with status eligibility.Their BGC are still pending. Note : The director sent them home and called another caregiver to come in.

Fire Extingguishers did not done annually at the operation .The last one was on March 2022.

The infants were watching television in the infant's classroom.

The operation did not have current Sheltering Drills for Lock Down Drill practice to review during inspection time.The last one was on Feburary 2022.

In the infant room there were 5 infants ages 5 months to 12 months old with one caregiver. The operation is over ratio by 1 infant. The director called another caregiver to help in that classroom, ratio was in compliance during the inspection.

During a follow up inspection on 09/02/2021, one staff did not have an employment affidavit to review.

The annual fire inspection was not available to review .

The caregiver was using screen time activity for the classroom have children under the age of two years. Note: I discussed with the director and the caregiver,the televison was turned off .

Caregivers did not serve lunch that matching with daily menu post in the center. Notes: The director wrote the correcting note in the menu.

Out of 6 satffs 's records evaluated, employment affidavit statement were not available in 2 staffs 's files.

Five of six caregivers' records reviewed did not have the required 24 training hours for the last full year of employment.
