The admission forms used were outdated. The admission forms do not capture the required minimum standards changes of March 2023.

The play structure's use zone was not free of obstacles that a child could run into or fall on top of and be injured. The play structures did not have the proper use zone.

The cots were not labeled with child's name or number/child assigemen map was not available. This was corrected during the inspection when the provider placed labels on the cots.

The children's files enrolled in care does not have a written parent notification of liability insurance. It was not posted or in the operational policy.

There was no documenation on the safety drills.

The provider received training on 03/18/2024 on CPR/AED: Adult, Child & Infant + First Aid (BLS). The documentation lacked pediatric first aid and pediatric CPR.

There was a trampoline in the backyard with an apparatus near the tramoline which may be used to climb onto the trampoline, and the netting was unzipped.

The swings did not have the appropriate use to and fro zone. The swings were set close to the fence.

The last documentation of the dogs being vaccinated was 11/10/20. They were due to be vaccinated by 11/10/21.

Two children's files did not have the immunization record on file.

The provider completed 20 of the required 30 training hours for training period 07/21/21-07/21/22.

There were 2 children's files that lacked the health statement from a health care professional.

The operational policies did not indicate that the operation has animals (dogs) in the home.

There was a trampoline in the backyard with removal step ladder attached, and the netting unzipped. This was corrected at the inspection when th provider removed the step ladder and zipped the netting.

The applicant lacks the training in (D) Understanding early childhood brain development; (E) Understanding the developmental stages of children; (G) preventing and controlling the spread of communicable diseases (H) administrating medication and (I) preventing and responding to emergencies due to food or an allergic reaction (J) building and physical premises safety(K) handling, storing, and disposing of hazardous materials

The operational policies were incomplete and/or lacked all the required information, reference standard 747.501. The operational policies lacked 747.501(6, 12), standards (9 contradicted the standard, 10 was incomplete, and 11 required additional informaiotn).

The applicant received basic life support in CPR and AED on 10/31/20.
