Two caregivers were observed administering prohibited punishment. One caregiver was observed pulling a child by the arm from underneath a table and another caregiver was observed grabbing a child by the arm and holding onto the child to keep the child from moving out of a chair. The same caregiver was observed grabbing on a child and pulling on the child's arms.

The operation has been out of ratio and failed to maintain supervision on more than one occasion. A caregiver with a provisional background check has not been prevented from being left alone with children.

The director failed to use good judgment when training certificates were issued for staff who did not complete the actual number of clock hours listed on the certificates of training. Caregivers completed fewer actual clock hours of training than are listed on the certificates of training signed by the director.

Based on evidence obtained throughout the course of investigation, caregivers are supervising more children than allowed by standards.

Based on information obtained during the course of this investigation, a child had access to garbage when the child was required to pick up garbage from the classroom.

Based on information obtained during the investigation, a child was left alone unsupervised on the playground for 21 minutes.

Based on evidence obtained during the course of investigation, a caregiver with a provisional eligibility has been left alone with children. The provisional background check stated the caregiver could not be left alone with children.

Caregivers in 2 rooms did not know the exact number of children in their room. In one room, the caregiver in one room stated she had 21 children but 22 children were present. In the other room, the caregiver stated she had 22 children but 33 children were present.

At the time of inspection, one caregiver in the school age room was supervising 33 children. The caregiver should not have had more than 26 children.

Daily reports for infants did not list a mood for the child.

Two rooms had cots which were not spaced to allow a caregiver to walk around each cot. One room had 5 cots closely spaced; the other room had 5 cots, 6 cots, and 3 cots closely placed to not allow any walking room.

A van used to transport children did not have a functioning electronic child safety alarm.

Emergency medical information for children being transported was not in a new bus used to transport children.

Five children records indicated the child had a food allergy, but there were no food allergy forms completed for 4 of the children, and one food allergy form was missing the parent's signature.

A licensing incident/illness form was not available at the time of inspection for a child who was injured in care and required medical treatment.

Daily reports for infants did not list a mood for the child.
