Two caregivers records lacked the educational documentation on file.

A classroom, prek 2, had 17 children with one caregiver. The specified age group (SAG0 was 3 years old. The classroom was over ratio by 2 children.

One of the van, the last row of the van lacked the latching mechanism. This was corrected at the inspection. A new seat belt was placed.

One child's record lacked an immunization record on file. Three children's records lacked an updated immunization record.

Lock down drills were not conducted in the 2023.

In the toddler playground, the 2 play structures measured less than the required 6 feet of use zone. The use zone overlapped. This was corrected at the inspection when the equipment was moved.

The wooden play structure had different areas of rotten wood.

This was last validated on 11/16/2022. This was corrected at the inspection.

Six infants in infant 1 did not have the written feeding instructions sheets reviewed and updated by the parents within the last 30 days. The most current one was last reviewed and signed in January.

The stationary equipment was moved; however, it did not meet the requirement as the use zone overlapped with other stationary equipment. The stationary equipment was moved during the inspection by the owner.

Three caregivers in the classroom did not have an active background check status.

There were 7 infants with one caregiver. The specified age group (SAG) was 15 months. With SAG of 15 months, the child/caregiver ratio was not met.

There were 3 stationary equipment which did not meet the use zone measurement.

The caregiver was using the operation's electronic device to submit information after the children completed having a snack. The children were engaged in free play.

The child/caregiver ratio was not met for the toddler classroom. The caregiver had 4 children on a buggy and 3 children in the classroom. The children had returned to the classroom from the playground. The specified age group (SAG) was 15 months. The child/caregiver ratio was over by 2 children in care.

Two infants did not have the written feeding instructions reviewed and updated. One was dated 06/08/21 and the other 08/01/21. One other infant did not have the written feeding instructions.

There were infants that were sitting at the toddler table in the bucket seats without the safety straps not fastened or sitting in a bucket seat that had no safety straps. This was corrected at the inspection when the infants were buckled in or moved to a seat with a safety strap and buckled.

Two children's records required the health statement from a health care professional.

A caregiver in the infant 3 classroom during breakfast did not have the eligibility status.

The emergency care authorization was not filled out by the parent.

Four children's records did not have the updated immunization record on file. Two children's records did not have the immunization record on file.

One child, who required the vision and hearing screening, did not have a vision and hearing screening on file.

Three caregivers had education documentation on file that was not translated to English.

Three caregivers did not have first aid training on file. Two caregivers were in direct care with children in the classrooms pre-k 3 and toddler 2.

One caregiver did not have education requirements on file.

There was one outdoor play equipment which was not anchored.

The children's records were incomplete.

Part of the toddler playhouse was dislodged from the base. Two stationary play structures, bicycles and school bus, had chipped paint. The older school age play structure had one slide with a hole on it.

The personnel and training records were incomplete.

The Children's Products form had not been reviewed annually. The last posted dated was 08/30/2019.
