The last annual health inspection was conducted on 04/12/22.

A fire extinguisher in a classroom was on the countertop and not mounted. This was corrected at the inspection when the director removed it.

An infant classroom had 6 infants with a specified age group (SAG) of 15 months.

One classroom did not have the emergency and evacuation diagram posted. This was corrected at the inspection when the director had it posted.

There were 7 stationary equipment sharing the same use zone.

The stationary play equipment was not anchored.

There were 3 individuals no longer associated with the operation with an active staus. This was corrected at the inspection.

There were 5 caregivers without a CPR training certificate in direct care of children.

There were 5 caregivers without a first aid training certificate in direct care of children.

The surfacing on the slided on pre-k/schooler playground had been compacted.

One child's record did not have the updated immunization record on file.

The required annual training on the following topics had not been completed by caregivers. The required training topics are 1. emergency preparedness 2. communicable diseases to include immunizations 3. administering medication 4.preventing and responding to emergencies due to food or an allergic reaction 5. building and physical premises safety and 6. handling, storing, and disposing of hazardous materials. One caregiver lacked 1-6, and 5 caregivers lacked #2.

The fence gate had a missing board. The fence in the playground had an opening at the bottom. This was corrected at inspection.
