A 10 month old infant was asleep on a play yard with an extra mattress placed in it, a blanket covering the mattress, and the infant on a pillow.

The first aid kit was incomplete. It lacked the guide to first aid guide and adhesive tape.

There was no documentation of the vaccination for 2 of the cats. The last documentation was from July 2021 requiring vaccinations July 2022.

The required annual training on the following topics had not been completed by caregivers. The required training topics are 2. communicable diseases to include immunizations 3. administering medication 4. preventing and responding to emergencies due to food or an allergic reaction.

The caregiver had not completed the required annual training for shaken baby syndrome, sudden infant death syndrome, and early brain development. The last time it was obtained was on 11/11/2020.
