The caregiver admitted to hitting a child on the hand.

The operation has only conducted two sheltering/severe weather and lockdown drills for the year.

There were six children in care missing specific admissions information. Two children did not have date of admission. Six children did not have an emergency contact address. One child did not have authorization to obtain emergency medical care. One child did not have physician or emergency care facility information.

There were several staff missing the following in their personnel file: No ID No Employment Affidavit No proof of education

In three of the classrooms there was plastic bags underneath each cot where children were sleeping. Note: This was corrected at inspection when the caregivers removed the plastic bags and placed them out of the reach of children.

There were several children in care missing the required admissions information: - no admissions date - no emergency contact information/address

The childcare center director did not have the 2 hours of annual transportation training.

25 out of 25 children who are transported did not have emergency medical transport and treatment information in the bus binder.

All fire drills documented for 2022 exceeded 3 minutes.

There were 3 out of 10 children in care with no physician health statement.

A caregiver with a hire date on or around 11/10/21 was present with children and did not have a background check requested nor background check results received. This was corrected at inspection when the caregiver left the operation and will not return until eligible results are received.

The use zone for the smaller play structure with a slide was only 3 feet on one side of the slide and 2 feet at the access of the slide. The required use zone for the slide (exit and sides) and access points are 6 feet each.

The people list was last updated on 12/28/2020. It should be validated every 3 months at least. This was corrected at inspection when the list was validated.
