In multiple rooms, there were baseboard liners which were missing or damaged. Paint was peeling from walls. Tiles were missing from the floor. Multiple toilets were not functioning. One sink was clogged/slow draining. There were holes in the wall in multiple rooms. Ceiling tiles had water damage and needed to be replaced. Outside, there were rusted paint cans fastened to the fence exposing sharp rusted edges. Nails were not flush with the boards on a music area and wooden playhouse structure. A fence board was broken along the back fence line. The red playhouse has wood around the frame which is broken/damaged. Wood around the music area is warped presenting a trip hazard. Several toilets had yellow stains and smell of urine along the toilet area; the walls behind some toilets appeared stained with a brown color. The sink area in multiple rooms had brown stains, discolored areas, and dried paint which needed to be sanitized/cleaned. Multiple lights were not functioning and need to be replaced in classrooms and restrooms.
A fire extinguisher in a room used by children was not mounted. It was stored on top of a counter.
A slide in the playground area did not have loose fill or unitary surfacing in the use zone area.
