The AM menu was observed to not be notated with the substitution for breakfast and AM snack. Corrected at Inspection: The director notated the menu with the substitution during the inspection.

In all classroom through out the building floors and bathrooms were dirty. Paint is still chipped off the walls. On the playground the unitary surfacing is still missing. The operation has placed a lock on the container with the filters.

Caregiver was unsure of three children in the classroom ages, caregiver did not have an updated roll sheet for the class. Corrected: When the front office gave the caregiver an updated roll sheet for the classroom.

In classrooms in the building the walls and floors were dirty. Paint is chipped off the wall. In the make believe boulevard classroom the ceiling tiles were coming down. On the playground the unitary surface had parts missing. A chest container on the playground did not have a lock on it for children to not have access to the filters in the container.

A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

Two classrooms were out of ratio at the time of inspection. The infant classroom was over ratio by one infant and the two year old classroom was over ratio by four children. Note: children moved classrooms when additional caregivers arrived to the center to correct the ratios at inspection.
