Four records were checked and the enrollment information was missing a few items. The fiie for one child was missing release information and authorization for emergency care. Another file was missing release information, no special needs and no authorization for medical emergency. One file was also missing information about the school the child attend and special needs information. The fouth child was missing school age information and physician information.
