During this investigation, it was determined the operation allowed a child to return to school without a doctor's note after being notified by the child's parent that the child had two communicable diseases (pink eye and hand, foot, and mouth) during different time frames. Note: The child is no longer enrolled at the operation. The de-enrollment date was on 11/28/2022.

The child's parent informed the operation that their child had pink eye (conjunctivitis) and hands, foot, and mouth disease. Note: The Director sent a notification to parents of children in the classroom where the child with the communicable diseases was present.

There is no documentation lock down drills are practiced at least four times a year.
The sink children use for hand washing and a toilet in the school age classroom needs repair. The sink is stopped up and is overflowing and 1 of 2 toilets are out of order.
Based on the information obtained, there is sufficient evidence to suggest that a caregiver administered prohibited discipline to a child in care. As of completion of the investigation the employee was immediately terminated.
