The caregivers do not have access to the tracking systems. The only tracking system that is used is signed by the parents and is located outside the front door.

1 of the 6 caregivers files I reviewed was missing orientation training at the time of the inspection.

The kiddie pools that were being used as water tables for the children over 3 years old were being used over a concrete surface outoors.

The toddler classroon, with children under 24 months, was using a screen time activity.

1 of the 6 childrens files I reviewed was missing the phone number and address of a emergency medical facility at the time of the inspection.

1 of the 6 caregivers files I reviewed is missing 7 hours of annual trainings at the time of the insoection.

Upon review of children's records, two children did not have current immunizations.

Upon review of personnel files, three caregivers did not have required trainings in the topics of administering medication and preventing and responding to emergencies due to food or an allergic reaction.

An annual fire inspection has not been conducted.

A child's enrollment application did not have a consent to seek medical treatment.

The caregiver failed to demonstrate competency when she physically forced a child's head and torso down on a mat and then arched her legs on top of the child to prevent him from moving. The caregiver is no longer employed by the operation.

A caregiver, was seen on video, roughly forcing a child's head and torso down on a nap mat. She is then seen arching her legs over the child to prevent him from moving. The caregiver is no longer employed at the operation.
