During the investigation, it was determined that a caregiver utilizes blankets with children under the age of 12 months to assist the children in falling asleep by covering their heads.

Caregivers did not properly count the children in care resulting in a 2 1/2 year old child being left outside on the playground alone.
The operation did not notify the parent of a situation that place their child at risk until the following day.
Caregivers did not provide adequate supervision to the children while on the playground. A child, age 2 1/2 years, was left on the playground unattended for approximately 3 minutes. The child was found by another caregiver and returned to the building unharmed.
Upon entering the building from the playground area, the caregiver did not count the number of children in care to ensure all of the children came inside and assumed all of the children were accounted for at the time. The caregivers did not have a method in place for conducting head counts at the time of the incident. As a result, a child, age 2 years 5 months of age, was left outside unattended on the porch area. NOTE: At the time of the investigation, the caregivers had developed a threshold count sheet system and post the number of children in care during the day to frequently monitor the number of children in their care.
