The wrong number of children was given by a caregiver in the classroom.

Two children were observed going to the restroom, but not washing their hands afterwards.

While putting on a music on YouTube for nap an advertisement played in two rooms while children were present.

A child was left alone inside a classroom while the other children and caregivers were on the playground.

A 13 month old infant was present in a classroom with the oldest indiviual 42 months older than them.

Screen time was being used at arrival with a 13 month old present in the room watching.

Only 4 fire drills were noted to have been complete in 2023.

A bottle of Lysol disinfectant spray was seen sitting on a cabinet within children's reach inside the classroom.

The wrong number of children were stated present in the classroom until it was mentioned to the caregvier.

A tracking system for children was not available for review during this inspection.

During the inspection the caregivers did not know how many children they were in charge of and had to recount multiple times.

During the inspection paint was observed peeling from the baseboards and wall exposing some drywall.

A child was observed wandering into the daycare office. Staff was unaware the child had done so.

During the inspection a bundle of bananas were observed in the food pantry to have mold on them. This was corrected at the inspection when the bananas were thrown away.

The nap cots were observed during the visit and were not labeled to indicate whose cot was whose.

Used diapers were seen in a trash can within reach of children in one of the bathrooms the children use. Children were seen using the bathroom. This was corrected at inspection when the trash can was removed from the bathroom by the director.

Used diapers were seen in a trash can with no lid within reach of children in one of the bathrooms the children use. Old food was also observed in a trash can within reach of children in a room where care took place.

A container of bleach wipes and baby wipes were observed in reach of children inside rooms where child care took place.

An infant was observed in a restrictive device for more than 15 minutes during the inspection. The infant was not being fed or transported.

During the inspection 4 children's records were reviewed and all 4 were missing the children's full address, the emergency contact address and 3 were missing the emergency contact phone number. 2 were missing the school's phone number, and 1 was missing the emergency facility or provider address.

During the inspection two caregivers with a pending background check were present and caring for children.

During the inspection 3 staff records were observed and found to be missing the statement of center policies, statement of personnel policies, orientation statement, employment begin date, and the age/education/qualifications.

During the inspection a 3 year old was observed in timeout for 5 minutes.

During the inspection the children started centers before washing their hands after coming inside.

During the inspection an infant was observed asleep in a restrictive device. The caregiver did not notice until it was mentioned to her.

During the inspection the person in charge did not know where the staff records were kept and could not get them to CCR staff.

The first aid kit was missing tweezers.

The operational policies were reviewed and found to be missing the Emergency Preparedness Plan. Along with the Safe Sleep Policy, Requirement for Hearing and Vision Screenings, Parental Notification of policy changes, Transportation, Water Activities, Procedure to review most recent report and standards online, and how to contact local licensing office.
