Three other employees were observed working in the capacity of caregivers and did not have an employee file. No files had been created for these employees.

During the investigation inspection, sufficient evidence was found to determine that proper supervision was not being maintained when two caregivers stepped out of their classrooms, leaving the children inside unsupervised. This was corrected during the inspection when the caregivers returned to their classrooms.
A caregiver was observed not washing the hands of a child. The caregiver carried the child back to the play yard without washing the child's hands.
Children were observed watching a non-activity related cartoon program on the television while eating snack.
During the inspection, a caregiver was observed using a changing station to change two children. The diaper surface was covered with a multi-colored non-absorbent material.
A caregiver was observed leaving the room to go to a hallway bathroom to wash the hands of a child, leaving 5 children in the room with one caregiver. The ratio of the room is 4 to 1. The caregiver returned to the room, bringing the room into compliance.
During the inspection one caregiver were observed leaving the classroom to answer a call from the director, leaving the children unsupervised in the classroom with the inspector.
Children were observed in two classrooms watching screen during the rest period.
During the inspection a caregiver was observed caring for children and was not in ratio. There were 6 children observed in care, bringing the room out of ratio by 2 children. The ratio is 1 caregiver to 4 children.
During the inspection an employee was observed supervising two classrooms of children from the doorway of the other classroom.
During the inspection, play yards were observed with blankets, toys and bumpy padding and hats. This was corrected at inspection when the items were removed by the caregiver.
During the inspection, 6 play yards were observed with mattresses that did not fit snugly the sides.
Children were observed during a designated rest period, with little to no lighting and the doors were closed.
During the inspection, a child was observed sitting in a restrictive device during the rest period. This was corrected when the caregiver was removed the child and placed her at a table with an activity.
A child was allowed to be in care at the operation without any of the required for a completed therefore not having any emergency contact information for the child.

Children were observed napping near the entrance/exit of two classrooms preventing the door from being opened all the way. One child was also observed napping underneath a table.

During the inspection it was found that the diaper changing mat in the bathroom had two small tears. This was corrected at inspection when a nap mat was used as the diaper changing mat.
During the inspection it was found that 2 out of the 6 children records reviewed did not have admission requirements completed and an up to date immunization record.
During the inspection it was found that the operation had a whole in the wall in the gym area that was accessible to children. It was also found that the operation outdoor play structure had cracks and a large whole with rough edges near the slide, and the foamed mats used for the outdoor flooring had large gaps.
During the inspection it was found that the two year old classroom and infant classroom had televisions playing. This was corrected at inspection when the television in both classrooms were turned off.
