The 2885 Children Product Certification form was not updated since 11/13/2023. This was corrected at the inspection when the inspector sent the form to the provider and observed the provider review and sign the form.

A caregiver left children in the classroom alone to open the door for licensing and parents.
During the walk through 2 play yards were observed with mattresses properly fitting the sides. 3 play yards had snug fitting sheets.
During the inspection the infant and toddler rooms were out of ratio. This was corrected when 1 caregiver was placed in each room, bringing the room inot compliance.
During the monitoring inspection a caregiver was observed making lunch for the operation, while solely responsible for the care of a classroom of 12 pre k children.
During the monitoring inspection it was observed that the television in the pre k room was not anchored or mounted.
During the monitoring inspection the children began eating lunch before having washed their hands.
During the monitoring inspection it was found that the transportation vehicle was not equipped with the electronic child safety alarm.
During the monitoring inspection it was observed that signs were posted preventing parent access to come fully into the operation.
During the monitoring inspection the transportation vehicle was not equipped with child safety seats and transported children daily between the ages of 4-7 years old.
During the monitoring inspection it was observed that the hand washing sink was being used to wash bottles.

The Director had not completed the required 30 hours of annual training.
A 7 month old infant was observed asleep in a bouncer, the caregiver stated he had been asleep in there for 20 minutes. The caregiver removed the infant from the bouncer and placed the infant in a playpen.
A caregiver was observed holding an infant's bottle while the infant was sitting in a bouncer.
Three caregivers did not have a notarized affidavit on file.
There were 5 infants, 6 months, 7 months, and four 12 month old infants in a classroom with 1 caregiver. An additional caregiver arrived to correct the ratio.
There were 3 caregivers that had not completed the 24 hours of annual training.
There were 2 caregivers that did not have documentation pre-service and orientation had been completed.
There were 2 caregivers observed in the building during nap time, each caregiver was observed counted in ratio in a classroom. Additional caregivers arrived during the inspection.
Broken glass was observed in the pathway to the playground, the grass was observed to be overgrown, there were 2 play structures that had cracks and 1 play structure that had a slide was observed to not have been placed on loose fill. The Director removed the broken glass, the broken play equipment and moved the other play structure onto the loose fill.
During the inspection it was found that the operation did not notify licensing regarding a communicable illness with in the required timeframe.
