During the inspection, the diaper changing area was observed with crumbs and dirt. This was corrected at the inspection when the caregiver wiped and sanitized the diaper changing area.

During the inspection, five trashcans were observed without lids. A toilet in the 4 year old classroom was observed without a lid on the tank. A toilet was observed with urine in it in one of the after school rooms. A ceiling pane light cover was observed missing in the 12 to 17 month old room. In the 3 year old classroom, chemicals were observed within the reach of the children. During the inspection, the director placed lids on the trashcans, flushed the toilet with urine, and placed a lid on the tank of the toilet in the 4 year old classroom. A caregiver removed the chemicals from within the reach of the children. The ceiling pane light cover was replaced during the inspection.

During the monitoring inspection it was observed that two of the sinks available for children's use did not have soap available for use. This was corrected at inspeciton when soap was added.

During the monitoring inspection toilet paper was not available for use in one of the classroom's bathrooms. This was corrected at inspection when toilet paper was added.
During the monitoring inspection one of the caregivers did not know how many children they were responsible for.
During the motoring inspection 8/18 children's files were missing part of the required admission information.
During the monitoring inspection it was observed that five of the classrooms were not following their daily schedule. While indoor activities were taking place, morning outdoor time for several classrooms were not occurring at the scheduled time. Classrooms were also doing activities different from what was scheduled during the inspection.
During the investigation inspection it was observed that a caregiver was changing diapers, one after the other, without washing their hands between each change. This was corrected at inspection when the caregiver was asked to wash hands between each diaper change and began to do so.
During the investigation inspection it was reviewed in the operational policies that the operation was stating that parent visitors were not allowed at this time due to COVID. This was corrected at inspection when the policies were updated while I was present.
During the inspection substitute caregivers in one of the classrooms were found to be lacking proper classroom management.
During the inspection it was observed that two sinks used for washing cups were also being used for hand washing.
During the investigation there was enough evidence to support the caregivers were not consistently demonstrating good judgment and self control while caring the for the children.
During the investigation there was enough evidence to support that the children were given long periods of silence or inactivity as a form of punishment.
