During the inspection an infant was observed sleeping in a bouncer.
During the inspection hand washing did not occur with every child when the children transitioned from outside to inside. This was corrected at inspection.
During the monitoring inspection it was found that a caregiver, whom had been employed greater than 90 days, did not have a CPR or First Aid training certification available for review.
During the monitoring inspection it was observed that the dirty diapers in the toddler bathroom were being stored in an uncovered container.
During the monitoring inspection it was found that many of the trainings for the staff were being provided by the director. Trainings hours for staff at this operation must not be through the director.
During the monitoring inspection it was observed that the floors and carpets were not being maintained in a clean manner.
During the inspection it was observed the operation had a toilet that was not usable and it was taped off. Child care regulation had not been notified of this prior to the inspection.
During the monitoring inspection it was observed that there was one caregiver with 25 children in a ratio that should have been 1/15. This classroom was over ratio by 10 children.
Upon arrival at the operation the children were observed to be watching TV. Four of these children were under the age of two years old,
During the inspection it was observed that the children were asked to stay seated at the tables for over 30 min, without any activities, while waiting for breakfast.
Upon arrival 23 children were observed to be present at the operation, combined as one group. The age difference between the youngest and the oldest was greater than 18 months, with infants combined with pre k children.
During the inspection it was observed that there was no soap available in the infant room for hand washing.
During the follow-up inspection supervision was found to be out of compliance. An incident occurred where a child put their hands in the toilet, and another incident occurred when a child got hand sanitizer in their eyes.
During the follow-up inspection it was observed that an infant, under 12 months of age, was sleeping in a crib, propped up with a pillow and covered with a blanket.
A caregiver was observed to be alone in a classroom without another caregiver present who had a cleared background with no conditions or restrictions. This was a violation of the provisional backgrounds for the caregiver present.
During an investigation there was sufficient evidence to support a child was abused while in care at the operation.
During the monitoring inspection it was observed that the daily grounds checklist was completed, but that not all of the hazards had been fixed. There was an outlet uncovered, a cleaning supply within reach of the children in care, a garbage can without a lid or a bag, and the playground had some trash items still present.
