During the inspection two ceilings fans located in classrooms used by children in care were found to be covered in dust. This was corrected at inspection when the fans were cleaned. During the inspection a caregiver's purse was observed to be on a shelf in an area where children were playing. This was corrected at inspection when the purse was removed and stored in an area that is out of reach of the children in care.

Children's potty chairs were in a classroom with waste inside of them. This was corrected at inspection when the potty chairs were removed from the classroom. Cleaning chemicals were observed on a bookcase within reach of the children in care. This was corrected at inspection when the items were removed and stored out of reach of the children in care. Garbage cans in two classrooms were found to have no lids and dirty diapers were on top within reach. The fence surrounding the playground areas were found to be rolling up and presenting an entrapment hazard.

The nap room was made up of mixed ages, specified age group for the room was 22 making the room out of ratio by 11. The infant room was found to have 10 children in care with one caregiver, making the room out of ratio by 6. This was corrected at inspection when an additional caregiver came into the room.

During the inspection two separate classrooms were observed to not wash the children's hands after changing their diapers.
The first aid kits reviewed in the transportation vehicle was found to have an ice pack that exploded inside, some items were missing, and had expired medication. This was corrected at inspection when a new first aid kit was placed in each transportation vehicle which has all required items in them.
Activity plans observed in the operation's classrooms did not include outside time for any of the age groups and were not detailed as to the activities the children engage in.
An operation staff member stated they place children in time out for 5-10 minutes, which exceeds the required minute per age for their age group. This was corrected at inspection when appropriate time frames were discussed with the staff member.
A caregiver, responsible for a group of children in care, was found to be asleep.
