A caregiver was observed pulling a small child up by the arm and sitting the child down aggressively on the bottom.

A trash can without a lid was observed in the classroom where children were being provided care. This was corrected when the trash can was emptied and removed from the room.
Parents were observed not being able to enter the rear of the building where children are being provided care.
Two caregivers were observed laying down near children during nap time. The caregivers were covered with blankets and had placed a small covering over the lamp used for lighting.
The operation had not practiced fire drills during the month of September.
3 new staff did not have signed notarized affidavits or the required pre-application affidavits. 4 of the 5 personnel files did not have enough annual training hours to evaluate, Staff records did not have completed preservice training that was documented.
The operation was found to have violated conditions that CBCU placed on a person?s presence at the operation when the person was found to be the only caregiver in a classroom with children in care.
The carbon monoxide detector test had not been conducted or documented. This was corrected during the inspection, when the detector was tested and documented.
The grass had not been cut. This was corrected during the inspection when the lawn man cut the grass. Broken outdoor toys were observed and corrected when removed from the playground. A broken urinal was observed, and a repair order was made. Lysol wipes was observed on a diaper station. There were no children in the room, and the wipes were removed.
Four concrete liners were observed overlapping the play area and in the use zones.
Cribs were observed with sheets that did not fit snugly on the mattress. This was corrected during the inspection, when tight fitting sheet were placed on the mattresses.
During the inspection, children were observed sleeping on the floor with no cot or mat. Children were observed sleeping on torn mats.
This standard has been reviewed and found to be deficient. the caregiver without malice or anger, did not use good judgement when picking a child in care up by the arm causing injury.
During the inspection, 2 children were observed asleep in cribs with loose fitted sheets. 7 cribs in total were observed with sheets that were not tight fitting.
All 7 employee files reviewed did not have training on the following topics: Administering medication, if applicable Preventing and responding to emergencies due to food or an allergic reaction Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic and Handling, storing, and disposing of hazardous materials
In a room with children ages 3-5 years it was found there was a bottle of bleach water stored in reach of children. In the school age room there was trash accessible to children. In a room with children ages 23 months - 3 years there was a caregivers purse stored on a couch that was accessible to children.
An employee was present at the operation prior to receiving an eligibility notification. The employee did leave the operation while the inspection was being conducted.
A caregiver in a room with 23 month olds - 3 year olds did nont know the ages of the children care.
During nap time a room with 2-3 year olds was too dark to see the children napping, therefore supervision could not be maintained.
5 children's files were reviewed, 1 was found to have no shot records, and one was found to be missing an MMR and a varicella vaccine.
There were 10 children 13 months - 20 months with a specified age group of 14 month and 1 caregiver in one room. 8 of the children were sleeping and 2 of the children were awake.
in a room with 14 month old children it was found that 3 children were napping with their heads covered by their blankets. In another room with children ages 23 months - 3 years 2 children were sleeping with their heads covered by their heads. This was corrected at time of inspection when the caregiver took the blankets off each of their heads.
Both Vans used to transport children do not have child safety alarms.
From interviews and the investigation it was found that the operation did not provide the incident report with in the required time frame.
During the inspection it was found that the transportation caregiver did not have annual transportation safety training completed.
