Out of the four staff files reviewed, none had a notarized employment affidavit on file.

The operational policies were missing procedures for inclusive services to children with special needs, enrollment and policy change procedures, criteria the daycare will use to decide when weather is too extreme for outdoor play, a plan for physical activity to occur indoors when weather prohibits outdoor play, that employee vaccinations aren't required, and the DFPS website.

Out of four staff files reviewed, three had not completed their 24 hours of pre-service training within 90 days of employment.

Out of 10 children's records reviewed, one was missing a statement from the parent that operational policies had been received, three needed updated immunization records, one was missing information on whether or not he had allergies, one was missing a hearing and vision screening, two were missing emergency contacts, one was missing an admission date, and two were missing a health statement.

During the inspection a caregiver did not wash the hands of the children after diapering.

During the inspection a non mobile child was observed asleep tummy down on the floor. This was corrected when the child was picked up and placed in a crib facing up.

During the inspection, a caregiver was present without a cleared background check. This person was observed to leave the operation.
