During the inspection the fire extinguisher was not mounted.

During the inspection children's records were reviewed and both files reviewed were missing admission information such as medical authorization, emergency contacts, and health statements.

During the monitoring inspection there was one adult present in the home, who is a household member, and they did not have a background check active in the system. Household member's background check was entered while I was present in the home.

During the investigation inspection it was observed that alcohol bottles, empty cans, and an ashtray were in the backyard, within reach of the children in care.

During the investigation it was observed that the children were not following the written activity plan that was posted.

During the annual monitoring inspection it was found that the fire extinguisher was being kept under the sink and should have been mounted.

During the inspection it was found that the operation did not yet have a fire extinguisher at the new address.

During the inspection it was found that the operation's living room TV was not yet mounted, creating a possible tipping hazard.

During the inspection it was found that the operation had not yet created an evacuation diagram for the new address.

During the inspection it was found that the operation did not yet have a first aid kit available for use at the new address.

During the monitoring inspection it was found that the operation had not practiced a fire drill since 12/2020. This was corrected at inspection when a fire drill was conducted while present at the operation.
