During the inspection it was found that the operation's transportation vehicle did not have an electronic child safety alarm installed.

During the monitoring inspection it was found that the operation did not have medical release forms on the transportation vehicle for the children who are transported. This was corrected at inspection when this information was placed in the vehcile.

During the monitoring inspection it was observed that the children in the 1 year old classroom were watching television.

During the inspection it was observed that the outdoor areas did not have loose fill where the slides, rocking equipment, or climbing equipment were.

During the inspection it was observed that there was a 5 month old infant in a pack n play with a blanket.

During the inspection it was observed that there was a 5 month old baby with a bottle propped up with a blanket.

During the inspection it was observed that there was no paper towels in the bathroom for the school age children.

When I arrived at the operation there were 9 infants, ranging from 5 months - 18 months, with no caregiver present in the room.

During the inspection it was observed that there were pack n plays that had a mattress that did not fit wall to wall, was not designed for the pack n play it was on, and there was added foam padding between the mattress and fitted sheet. This was re-cited.

During the inspection it was observed that a caregiver sat down to feed an infant their bottle and did not wash their hands first.

During the inspection it was observed that it was 72 degress and sunny outside, however the children had not been outside the day of the inspection. The playground was not muddy and the play equiptment was not wet.

During the inspection it was observed that there was a changing table in the pre k room with no sink in this area.

During the inspection it was observed that there was a swing in the toddler playground that was attached to a play structure.

During the inspection there was a caregiver with a provisional background that stated they could not be alone in ratio, this caregiver was alone with a group of 9 children, ages 2-4 years old when I arrived at the operation.

During the inspection it was observed that the bathroom in the school age classroom had cleaning supplies on the floor and on a low shelf. There was also clorox wipes on a shelf in the room next to the snacks that the children were being served.

During the investigation inspection it was observed that there was pack n plays that had a mattress that did not fit wall to wall, was not designed for the pack n play it was on, and there was added foam padding between the mattress and fitted sheet.

During the investigation inspection it was observed that the floor in the school age room was not in good repair and due to this not able to be effectively sanitized.

During the investigation inspection it was observed that a child did not wash their hands, and was not assisted on washing their hands, after their diaper was changed.

During the investigation inspection it was observed that a caregiver was changing diapers, with gloves on, however wasn't washing their hands after taking the gloves off and before putting new ones on.

During the investigation it was found that the operation was using forms of prohibited punishment by having the children do wall sits, and other forms of exercise, as a way to control their behavior.

During the investigation it was found that there was a child in care who was allowed to use cleaning supplies at the operation.

During the investigation it was found that a person was present at the operation, in a caregiver role, with a background that stated they were not eligible to be present.

Observed 11 infants in a classrooms with 3 caregivers. The specified age for the group of children in that classroom is 0-11 months. The maximum group size for 0-11 months is 10 children. This was corrected when a child was removed from the room.

During the inspection there was standing water observed in multiple riding toys in both the toddler and Pre-k playground areas. There was liter observed on the Pre-k playground.
