Inspector observed one staff with 7 infants between the ages of 3-17 months old. There was no extra staff observed at the facility during nap time when the inspector arrived.
The operation's last fire inspection was dated 6/5/2023.
Staff in building #2 stated no fire drills had been conducted in at least a year. No documentation of fire drills was found for 2025. The operation also did not have documentation of severe weather and lock down drills in building #2.
Two out of four staff records were not available for review at the time of inspection.
Inspector observed two infants asleep in restrictive devices. One infant who was 8 months, was in a swing and the other infant who was 11 months was in a stationary walker. Inspector spoke to caregiver and infants were removed. At the end of inspection, inspector observed a 3 month old asleep in a swing.
Two out of four staff were missing annual training hours. One staff was missing all trainings in section (e) of this standard as well as 7 out of 24 annual hours. The second staff was missing 5 out of 30 annual hours. The hours missing included hazardous materials and management training.
Two out of six children's records did not have the required documentation available for review.
Inspector observed one caregiver supervising two classrooms during nap time with no additional staff. Two children ages 3-4 yrs. old were in one classroom and the caregiver was in a secondary classroom with infants who were between the ages of 3-14 months old.
Five out of six children's records did not have immunizations on file.
Inspector observed one-3 month old infant in a playpen lying on back with a mobile toy. The infant was placed under the toy so the infant could play with the objects above.
The infant's daily schedule does not indicate that infants go to play outside twice a day on a daily basis. Staff stated infants do not go outside to play. There is no infant playground located at the operation.
The operation had one carbon monoxide that was not working in the pre-k building. The operation also had no documentation of carbon monoxide, smoke detector, or fire extinguishers being checked monthly.
In the infant classroom an infant was in a play yard with a training cup toy and a blanket. NOTE: This was corrected at inspection when caregiver removed the items from the play yard.
A caregiver did not show good judgment by taping a child's shoes, so the child would not remove the shoes.
During nap time in the infant, toddler, and Pre k classroom the cots, and the play yards were not arranged with space between them to allow caregivers to walk between them.
The infants were in the room designated for the toddlers and the toddlers were in the room designated for the infants. Director had not notified licensing of the changes of the classrooms.
There were 6 gallons of milk that were not stored properly in the refrigerator, the gallons of milk were room temperature.
Base on the DFPS investigation; There was neglectful supervision at the operation, a child was left outside from 10 - 20 minutes with out supervision.
One out of four records review was missing training hours.
One staff member was observed using poor judgment by sleeping while children were also napping. Staff members also had music playing loudly during nap time.
At the time of inspection, the director was not present and staff were unable to tell the Inspector who the staff member in charge was.
One child at the operation had a food allergy written in their file under the special needs section of the admissions form but did not have a food allergy form filled out on file. There was also not a posted food allergy form posted in the operation for the child in the kitchen or their classroom.
Inspector observed two staff not properly supervising children during nap time. Staff were observed sitting in a corner of the room. There is a small cabinet where two children were observed sleeping and cannot be easily seen. Inspector also observed one staff sitting while allowing another child to sleep with a blanket on their head.
The operation did not have documentation of testing the smoke detector, carbon monoxide detector, or the fire extinguishers for 2020 or 2021.
