During an inspection it was found that an infant classroom had 11 children in one classroom with two caregivers. This was corrected at the inspection when the infant was moved to another room putting the classroom into compliance.
It was found through an investigation that parents were not informed of the actual injury their child sustained in care.
7 of the 9 infant feeding instructions reviewed were more than 30 days old or did not include a date.
The caregivers did not demonstrate good judgment when the caregiver's wrote an incident reported that stated that a child was walking and fell on a toy. The investigation showed the child was placed in a crib and fell out. The incident report did not match the incident that occurred.
A child was placed in a crib that did not have a safely secured drop down gate, causing the child to fall out.
The operation is found to have a class of 18?23-month-old children over ratio.
During inspection a nine month infant was observed to be sleeping in a crib with a blanket. This was corrected when the director removed the blanket.
During the inspection a school age child was observed wandering around the operation alone. The child was looking for a caregiver unsupervised until a caregiver came and got the child.
The director did not designate a caregiver in charge in her absence.
During the inspection caregivers were observed walking out of classrooms for up to three minutes leaving the classrooms out of ratio.
During inspection, in the Publisher classroom, there were 15 2yo-3yo children outside on the playground with both caregivers. There were two children observed walking away from the group and stood by the playground entry door. One child was observed climbing the window seal and running inside. The caregivers were unaware the two children left the group. The assistant director informed one of the caregivers to retrieve the children.
It was found during the inspection that a caregiver said they had 13 children in the classroom and there were 17.
During a walk through of the operation, it was observed that a caregiver left the classroom to assist a parent in the lobby. Leaving that classroom out of ratio.
When a caregiver was asked how many children they had the caregiver said they had 16 when there were 17. There were two caregivers in seperate classrooms that gave the wrong number of children in care. One caregiver said their room had 12 when they had 11 and the other caregiver said they had 17 when they had 15.

It was found while conducting the inspection that the operation had left a caregiver in charge of a group of children with a provisional background check.
It was found when I arrived at the inspection that a single caregiver was with 17 children with the specified age group of 3. The operation also exceed the max group size when another caregiver arrived and they were left with 25 children with a specifed age group of 2.
During the investigation it was found that a child was not being adequately supervised when they fell off a slide causing three fractures in child?s arm. The caregiver admitted to not having visual/auditory supervision of the child during the incident and stated that they were unable to intervene to ensure the child?s safety.
Parent was not notified that child had wondered out of his room unattended, on past occasions.
On serval occasions caregivers allowed child/children to leave asssgined room. In addition, director took eyes off children, allowing for children to lock director out of office.
Childcare Regulations was not notified of child/children leaving the room on past occasions.
