The Spring Rider and Circle Cycle structure were observed with rusted area. There was an unused water heater outside in the outdoor play area. There was grass growing from the mulch. There was no mark for the mulch to indicate the depth of the surfacing material.
A 4 seat Spring Rider did not have the required use zone area.
The Infant Caregivers did not have the following annual training topics: (1) Recognizing and preventing shaken baby syndrome and abusive head trauma; (2) Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3) Understanding early childhood brain development. The person in charge of transportation did not have a transportation training in the file for review. A caregiver hired on 12/16/19 did not have Pre-Service training certificate in the file. Several training certificates did not have an assessment tool to show the knowledge acquired from the trainings.
A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

The following required personnel records were not available for review: -Documentation showing educational qualifications for one caregiver. -A notarized Licensing Affidavit for one caregiver. -A statement signed and dated by the employee showing he has received a copy of the child-care center's policies for two caregivers. -A statement signed and dated by the employee verifying the date the employee attended training during orientation for two caregivers.

Two children were observed inside cribs assigned to other children by name. Both cribs were not labeled with children's names correctly. NOTE: This was corrected during inspection. Correct names were added to each crib.
4 infants were left unsupervised during nap time in the classroom as caregiver exit to use restroom. There was a floater caregiver at the operation but was not notified in order to offer coverage. NOTE: Caregiver returned from restroom break and supervision was resumed. Administrator talked to caregiver about supervision.
Children's products certification was not completed timely. Last completed form was on 7/2019.
